What's the correct way to write an "Honor Code Statement" on a paper.

I'm a transfer student and I never really learned (I've gotten away with just a signature) but now I have a very detail oriented prof who wants the right cover page for every assignment. This is for a paper submitted via Sakai, so there won't be an actual signature. What do I put instead?

Allegheny College's Honor Code Statement can be found at http://sites.allegheny.edu/about/honor-code/.

Generally, the honor statement is placed at the bottom of the title page, following format, font, and size of the other required items. Be sure to ask your individual professors on what to include.
